June 2018 - Authorised Prescriber applications

The Eye and Ear Hospital has updated its procedures for Medical Practitioners applying for Authorised Prescriber status to align with the updated guidelines issued by the Therapeutic Goods Administration in July 2017.

The major changes are:

  • A revised Eye and Ear Hospital Authorised Prescriber Application Form
  • A new Informed Consent form for use of unapproved medicines and devices
  • A new procedure flowchart
  • Clarification regarding the procedure

Medical Practitioners are reminded that: 

  • Unapproved Goods can only be prescribed by a Medical Practitioner with an Authorised Prescriber permit issued by the TGA
  • Informed Consent must be recorded on the medical record
  • Adverse Events must be reported to the TGA 
  • 6 monthly reports must be sent to the TGA
